​Compared with paediatric healthcare providers, adult healthcare providers often have less time to spend with each patient. As you attend more appointments on your own, it is important to prepare a summary of your medical history so you can share it with your healthcare providers concisely. This skill will help you to communicate your needs and concerns in different healthcare situations.

The MyHealth 3-sentence summary​ gives some tips on how to summarize your health and current needs in about three sentences. Once you can summarize your history, you briefly send the message that you know about your health and know how to focus on what is important.

After you give your summary, your provider will typically have questions. This doesn’t mean that you’ve forgotten important information! It just means that you’ve given enough information to let them focus on what’s important to you during the appointment.

What information goes into the health summary?

To be as concise as possible, your summary should contain the following information:

  • Sentence 1: Your age, diagnosis and brief medical history
  • Sentence 2: Your treatment plan
  • Sentence 3: Your question or concern to talk about during this visit

Sample summary

Hi, my name is Anita Patel.

  • Sentence 1: I am 19 and have chronic pain - I was diagnosed when I was 16.
  • Sentence 2: I have been taking Tramadol for the past two months, go for weekly pool therapy sessions with my physiotherapist and practise mindfulness meditation.
  • Sentence 3: I am here today because I’m concerned that my medication is causing me to lose a lot of weight and I want to talk to you about options to help.
